Support at Home

Support at Home

In-home aged care through the Support at Home program, helping older people maintain daily routines, independence and community connection.

Support is shaped around the older person’s assessed needs, preferences and home routines. The agreed service mix may include everyday living assistance, support for independence and appropriate clinical services. Family and carers can be involved with the person’s consent. Before commencement, the service agreement explains what is available, who delivers it, how care is coordinated and how costs and changes to support will be discussed.

Support may include

  • Everyday living assistance, such as cleaning, shopping and meal preparation
  • Support for independence, including transport, appointments and social connection
  • Personal care and approved respite where included in the service arrangement
  • Clinical services only where qualified practitioners and an appropriate delivery arrangement are confirmed
