A transition from hospital can involve several people and services. Practical support helps clarify what needs to be ready in the community, who is responsible and how information will be handed over. The person’s preferences and consent guide coordination. The hospital retains responsibility for clinical discharge planning and treatment. Community supports begin within an agreed scope, with arrangements for follow-up and any change in needs.

Support may include

  • Coordinating funded disability supports after discharge
  • Liaison with hospital teams and community providers
  • Planning practical routines, equipment links and support handovers
  • Follow-up on agreed community arrangements
