Regional and Remote Disability Outreach

Outreach that brings agreed disability support closer to people living outside metropolitan service areas.

Outreach combines agreed visits with a realistic plan for follow-up between appointments. Planning considers the local service context, cultural preferences, travel, access and the availability of people who can help maintain continuity. The service area and visit frequency are agreed case by case. Where remote follow-up is suitable, it can support progress between visits without replacing an in-person assessment when one is needed.

Support may include

  • Scheduled assessment and support visits
  • Training for local families and support teams
  • Coordination with local providers and community organisations
  • Remote follow-up where suitable
